Patient Referrals
Anyone can make a referral.
Once a referral to Arizona Care Hospice is made, with the patient’s permission, we will contact the attending physician to identify if the patient qualifies for hospice care.
If the patient qualifies, we will work together to create a personalized care plan that will manage symptoms and provide emotional and spiritual support.
